Edit: Wait, what do you mean by "remove" in 2? Refunding? If that's the case, I'm not 100% on whether the achievement will remain.
2. Also correct, does not matter if refund or voluntary removal, achievements are linked to your profile once unlocked, it does not matter if you own the game anymore.
3. Also correct, you need to unlock at least 1 achievement in a game for it to count for the global achievement average calculation.
4. Honestly no clue but it should count since its unlocked achievements.
You only need to be eligible for achievements to count towards the global percentages. You don't actually need to have any achievements, just start the game. That's the reason the first achievement you're guaranteed to unlock in a game can have less than 100% completion rate.
I am pretty sure this is not true from my own experience and what i read by users who actively hunt achievements. Its true that the achievement statistic internally triggers for a game when you first start it but no stats get added to the global average till you unlock at least one achievement.
So the count of the achievements I got in that game still shows up. It doesn't show the specific achievements, but it kept the number :)
The global percentage is indeed based on how many players started the game, not earning the first achievement. I figured out when achievements were added for Risen.
