Do you want the Japanese sense or the western sense of the word?
The Japanese sense of it is essentially a woman (or girl) who acts and somewhat looks childlike, another term for them was burikko in the 80s. It has nothing to do with their age in terms maturity or being of age to consent to certain acts. Or, for that matter, the size of their breasts. There is a "large breasted loli" niche, too.
The English sense of the word confuses it with the term used in reference to a prepubescent child.
And we all exist due to reproduction (even test tube babies, etc., started from fluids, etc., extracted from humans. So sex is literally in everything around us. Without none of the world would exist.
What about it? That and Nekopara aren't 18+ games so they aren't comparable. Again your blindly taking their word for the reason. Nothing to do with the flat chest and its about the char appearing to be a minor.
That includes more then just how the char looks and can involve the setting, clothes, text, etc.
Nekopara is an adult game in its original form.
For that matter the original artwork (prior to the development of the games) was originally featured in an eroge magazine, too.
it still is.....but as far as im aware the game(s) you can buy on steam is the All Ages Version.....not sure if the Uncut dlc is still sold on steam or only on the third party publisher site.
Not on steam however, so it doesn't matter in this case what it originally was.

Because, as I said earlier, there are at least two definitions of the word. The Japanese usage and the western one. The lines blur because a certain group of localisers insist on forcing the western usage of the term whenever they find it used in Japanese experiences.
There's specifically a patch for the Steam versions though. And Valve know of the existence of these patches. For that matter, Valve have refused to allow certain games to advertise their patches while allowing others to host the patches directly on Steam.
The simple answer to why (and this doesn't just affect adult games*) is that Valve do not have a standardised approval process. It all falls down to one person making a decision and those decisions often contradict.
* Valve are the only platform holder to demand a Japan only port of an "all ages" console game from Koei Tecmo censor a non-sexual scene, while the other platforms (Sony and Nintendo) both allowed the scene uncut.
