Framerate not capping properly still
Had this issue basically since I've begun using the application, best way I can describe it is when I use Lossless Scaling on any game there are times where the App simply stops running correctly and starts outputting any framerate it wants.
If I hard cap at let's say 60, and regardless of if I use x2 or x3 it should be 60/120 or 60/180 but sometimes the game will simply start going above where my FPS cap like 147/120 or 147/150 which causes severe input delay and makes the generated frames feel very slow, I have no clue as to how to fix it.

Make sure that every overlay is disabled (Steam/AMD/Nvidia/Discord HW acceleration/stuff in your system tray), after you disabled everything reboot your PC.

I have all external overlays disabled, Nvidia,Steam and more, and this only happens sometimes not all the time, I'd say about once every 5-10 mins usually, however it's extremely easy to recreate in some games such as Starbound or Noita, when I pause the game and create a static screen it does it 100% of the time

Try running the games in different display modes (windowed/borderless/fullscreen), and different APIs if supported (DX11/12/Vulkan).

Fullscreen for me doesn't seem to work, but I've attempted Borderless window (which works best) then windowed mode which can have some scaling problems, but across all mediums it happens regardless, I've tested it in Cyberpunk, Insurgency Sandstorm, Starbound, and Deep Rock Galactic

Try a different GPU driver, uninstall it with DDU first.

It's an overlay, but it’s often hard to find as for example when Windows displays one when you adjusting volume. Or, you might receive a notification from a messaging app, such as a new message, which is also an overlay.

I see , so any notification could cause it such as but not limited to things like Discord, Steam Messages or Windows Notification, which all must be disabled?

It depends, Steam often renders its UI into game's backbuffer so it doesn't count as overlay. Same goes for RTSS. Other apps most often use overlay windows.
You can also try WGC capture and see if its affected.

I tried to use WGC and had issues with frame latency, I haven't given it a go since LSFG has come out, I can can attempt it
Yo! So I had the same issue and finally figured it out, if you have nvidia or amd instant replay (specifically desktop recording) functions on, it interferes with lossless and can cause this issue. Took me awhile but finally figured it out today
Disabling discord overlay worked for me.

Have the same issue. Before it works for me even with RTSS, HudSight and Shadow Play at the same time. What could have happened that now doesn't work at all?

On my case evey time I apply LSFG it just lowers my FPS to my max refresh rate even if they are WAY higher than that not talking about the typical drop. Every type of sync is disabled. No idea what is going on.

Just to be clear, this issue still persists to this day, I've tried listed fixes but have had no luck

Same issue, no overlays are on. Force disabled gamebar even.
