There are some playstyles that are a bit more active than others based on non-permanent choices you can make that can make clicking more relevant if you choose, but honestly I haven't needed to click the orb in months.
Certainly once you have all of the upgrade achievements, that's true. They come pretty slowly when you've played for a while, so 'just fifteen more' could mean weeks of progress. (current top is 2000 upgrades, so 15 is 0.75%.) It takes me less than a few minutes each run to top these out. Essential? No, but which one of us is wasting our time?
The fifteen upgrades in question are those that require manual clicks in the current run, which increase shard pool accumulation time. I'm not honestly sure any class really cares about that.
The only situation I can think of that (sorta, kinda; not really) "requires" manual clicking is void entity collection for Archon. Even then, it only saves you about an hour of less efficient build-up (out of 12+ hours, typically), and if Void Automaton wasn't so limited that wouldn't be a thing either. Again, some kind of external auto-clicker (mouse macro, AHK script, whatever) will make short work of it.
Indeed, the game is not designed around clicking, you just get started by clicking a bit in the very first runs and can just never click ever again without any negative impact.

200 clicks should take about two to three minutes, tops. this gets you access to 15 cheap upgrades for you total upgrades count. You could wait until you are close to a number of upgrades achievement, then do the manual clicks. I do mine early each exile, then ignore it until the next exile.
If they go away, no big loss. For the upgrade count, they are worth doing while they exist. Unless you like to click, they have little other impact on the game.
