What is Nvidia reflex low latency and should it be on or off?

With it off, your machine can generate frames ahead of the current one, but it doesn't take account of any player actions you might perform so it feels more laggy. Having it on reduces your framerate but feels more responsive.

It's a matter of preference, but if your performance is good you probably want low latency on. If your performance is not so good, you probably want LL off.
From Nvidia Control Panel (where you can also enable it for other games):

Description:
The Latency control reduces latency by limiting the number of frames the CPU can prepare before the frames are processed by the GPU.

Typical usage scenarios:
  • Select Off to prioritize render throughput by allowing games to queue frames.
  • Select On to prioritize latency by limiting frames to 1.
  • Select Ultra to prioritize latency by fully minimizing queued frames. In addition, this mode minimizes VSYNC latency when both VSYNC and G-SYNC are enabled.

I believe the in-game implementation allows AMD users to also use this option. I would assume it is the same as using the ON setting in NvCpl.

To be honest, i can't tell much difference either way. I think my performance is as good as it can get, considering the games poor optimization. It seems not even a 4090, the best GPU money can buy, is good enough.

If you can't tell much difference, don't worry about the setting for this game.

Not too surprising-- Hogwart's doesn't demand as much twitch reflexes as FPS shooters or metroidvania-style games with very tight jump timing and such usually do. LL mode matters more for those games.
