What, beyond having a full inventory of actual items, could cause a "Gear Slots Full" problem?
Op is mixing up actual gear and the cosmetics
"Gear" are items that you can equip. In the beginning, you can carry up to 20 different gear items. This number can be increased by completing challenges in your field guide that are related to Merlin trials. If you (say) click on your gloves, then the game will show you all the gloves that you're currently carrying, plus a number like "15/20". That doesn't mean that you're carrying 15 gloves, it means that you're carrying 15 gear items in total, and if any of them are gloves, then they will be shown in the grid below that number. If the first number is lower than the second, then you should be able to pick up more gear - if you aren't, then that's a bug.
"Skins" are cosmetics that you unlock. The number of skins that you can have, is not limited. Every time you pick up an item, its skin gets unlocked for you. You can destroy or sell the item, its skin will stay unlocked. When you hover your mouse over a gear slot in the gear screen, you'll see information in the lower right corner that you can press a key to change appearances. Press that key, and you can make the item that you're currently wearing look like any other item whose skin you have unlocked. This will only affect the looks of the item, not its function.
