When they say "every enemy" are we talking all unique enemies? For example, if I forgot about doing it to a Main Story boss, which clearly won't appear again, I have to start again?

No, its not for unique enemies (Infamous Foes on the map) or story-bosses.
Just the regular baddies.
But might as well use it on everyone you encounter, not that hard!

Someone on youtube mentioned Acromantula, don't see it on this list. I'm not sure but you can try just in case. There was one in the north battle arena.

There are other trolls too like Armored troll is that not one of them? What about the Infamous Foes? I know I 100% hit all of the ones you listed and did not get the achievement, I am at 94%.
I even got the Acromantula and the trial soldiers, Ranrok etc. I know that I did miss a couple infamous foes, not realizing I had used Petrificus Totalus and killed them, then realized I did not use AM on them. I also, used AM on Deaths mongrels and his fighters as well.

You are correct, there are also armoured trolls, as well as fighter trolls and fortified trolls. If you haven't gotten it yet, I'd try looking for those.
