dungeon puzzles backwards or dont reset properly

anyone else experiencing issues with dungeons where the puzzles work backwards from what appears to be intended (ie, solving the puzzle bars you from reaching the chest) and then once "solved" stop working and give you no ability to get the chest (without reload)?

I haven't noticed that, and I've done most of them...

i have one of those puzzles with the blocks that i wanted to reset but didnt know to shoot the orb so i backed out and now the blocks have shut the entrance so i cant get back into the puzzle

i had two do this. the one where you pull the handles in a pattern (which actually closed the gate when I completed it, the gate was open when i entered the dungeon)

and then the one with the column "jail" was already open when i entered the dungeon and putting the floating cube on the plate, just lowered a few other pillars but i could already get to the chest before that point.

so basically, if i *didnt* solve the puzzle, i just got the chest right away. so weird.
