Witch's Tomb Missing Moth Glitch?

I am in the witches tomb and I've reached the second door which requires 3 moths to open. There were two moths in the main area where to smaller tomb is and I blew the doors open and there was no other moth in there .... what's going on? Missing a third moth and it is no where to be found. Blew everything up, Reveliod everything and still no moth. Lumos is on obviously. Anyone else encounter this? I'm going to have to reload I guess.

1: Put moth on swirly thing.
2: Go on top of the platform.
3: Use repulso on swirly thing.
4: Pick up the extra moth.
(If it's where I think you are.).

That's not where I am - I had to restart the quest as it definitely glitched out. Second run was fine.
