I think that the free name change (and uniform change without visiting the tailor) you get is when you first accept them. As such a 'limited time offer'.
Tovan Khev (for Romulan) and the other factions (except the Jem'Hadar) have one similar - as mentioned, the very first BOFF you get when you start the character.
But also (not mentioned), some of the hologram / clone / whatever BOFFs that are named characters, also can't be renamed.
In all of these cases though, the rename option would simply be locked out. If you're able to open a rename window and it just won't take, it's likely that the name you are trying to use is hitting the filter list.
And usually renaming BOFFs doesn't have a cost.... so now that I'm thinking, I'm wondering if what you're doing is actually a BOFF rename. Is it possibly trying to rename your *captain*? Or are you in the tailor settings, and the cost might be costume pieces you haven't unlocked?
