At least personally, I never had an item just disappear.
For space items, I suggest checking to see if they are equipped on another ship - under certain circumstances, you can't access them, so go to a ship selector and look through your ships there. Same with the bank - if items happen to be in the bank, you can only use / equip them while the bank window is open etc. But unless you are in sector space / on a mission, you should already have access to items equipped on other ships - still, can't hurt to double check.
That's the only thing I can think of, really.
For everything else, you might want to check the overflow bag - perhaps something happened so your items landed there.
If they are really gone, you may have accidentally recycled them in the replicator. I know I have to pay attention when I mass discard stuff after runs, and on occassion had to reclaim something when I was in a hurry and deleted stuff I wanted to keep.
Hope it's not that (because the items are gone when you don't reclaim them in the same seesion), and you say you are fairly sure you never discarded them. Still, it's a possibility. Again, I have never had items just disappear - they where always somewhere I didn't quite remember putting them.
So I'm sorry I can't be more helpful - never bugged out me like that, if it's a bug.
