The DLC adds content in the middle of the campaign. So I would recommend playing it with the DLC from the start..

You can also play the DLC content as standalone from the menu - but I would imagine playing it as a part of your main playthrough would be more interesting :)
The DLC is mostly integrated with the main campaign so always turn it on. There is a standalone arena mode as well...

Also when you start a new campaign consider turning gladiator companion to off; you will still recruit them but they will be a tank class principe instead of a gladiator, this is because the gameplay is based around having at least 2 tank class principes in later missions and the gladiator is an advanced class that takes alot of skill to use.

Note the first gladiator you will meet in the story is always a scout class velite. The class conversion happens to the final companion.

It always bothered me that Bestia, whose entire identity revolves around being a gladiator...is not a gladiator class.

Or Deianera, who is from Scythia, isn't a sagittarius. Or Calida, whose unique ability is a melee takedown, is a sagittarius.

This DLC is a must, totally worth to buy if you finished first playthrough and want a second one. Anyway classes in game do not feel ancient and roman or greek, gladiators are an exception
