Combat??

How the hell does the counter system work, I've tried RMB when the red bar hits the blue section, I've tried LMB when the red bar hits the blue section, doesn't seem to matter, either way I get hit. What the hell is going on??

Honestly not even worth figuring out. Just get a sword and one-tap everything

Your answer makes me sad since I came to ask same as OP, but I appreciate knowing now.

Do the bladed weapons actually kill them or just stunned for a time?

there's no way to kill an NPC yourself; no matter what you do, they'll get back up in a few minutes.

The combat system is just really annoying and not fun to deal with, hence the "grab a sword and one-tap everyone" approach.

eh. roughly. two hits most, some three.

difficulty affects this though. thats the only thing it does actually

*edit: to clarify usually the sword is a one tap but on hard some enemies seem to take an extra hit.

Fist fighting is not worth it, because even if you best them they just flee and trigger the alarm. Police batton is a 2 hits solution. Sword is overkill.

no such thing as overkill when the npcs just fall over and get back up after 60 seconds

So, if you block when the indicator is in the blue, it breaks their stance and stops them from attacking for about half a second - unfortunately, it also stops you from attacking for about the same length of time, so that mechanic is currently useless. You CAN win unarmed fights one-on-one if you just keep punching: you'll probably be bruised, bleeding and have broken bones by the time they go down, but winning is entirely possible.

Don't waste your time hoping the Tenacity sync disk will help: the unarmed damage upgrade path is either broken or underpowered.

So, yeah, get yourself a truncheon or a sword or something.

If you press RMB while you're in the light blue zone, you'll block the attack and they'll be briefly stunned for a second. If you land it in the darker zone at the end, then you'll do the above and get a free punch with it.

As others have said, it's much easier to get a truncheon or a sword and use those instead. Or do it my favorite way, find a heavy object like a chair or a barrel and throw it at their heads.
