It seems that binding "Zoom In (Not "Zoom")" to "1 (Not Num 1)" causes key bindings to reset to their defaults in Halo 2.
Halo Reach and Halo CE have no issues with "Zoom In" being bound to "1". Probably explains why there aren't many other people complaining.

Delete gameusersettings.ini
Relaunch the game (which regenerates the file).
Rebind your keys and save. And close/exit and see if it resets when you relaunch again.
This didn't have anything to do with it in my experience. I had Zoom In/Out bound to Mouse Scroll Up/Down.
I'll try the GemUserSettings.ini reset to see if that works for me.

It's a great story but didn't take me too long to finish after this post.
This advice did not help me. Before, Halo 2's key bindings wouldn't save for me. Now (after deleting gameusersettings) none of the games key bindings would.

This works! I can now save Halo 2's bindings without issue. However, this seems to be true for Reach and CE as well (for me at least).
I use shift for zoom in so it's not just setting it to the #1 that is causing it.
I'm also having trouble binding voice chat to another key. I keep getting a message saying one or more commands aren't assigned.
