The question has been asked several times (today, even), and the answer has been the same every time: co-OP is limited to two players.
Ce and 2 and only two-player co-op.
3, ODST, 4 and Reach are 4 player.
Your recollection is flawed.
My recollection is also based off the xbox and xbox 360 versions of the games...
Halo 2: Anniversary – 2 players max
Halo 3 – 4 players max online
Halo 3: ODST – 4 players max online
Halo: Reach – 4 player max online
Halo 4 – 4 players max online
Halo 5 also had three others characters (6 additional total considering both teams), but they were always there in narrative & gameplay from what I've seen with that revive & team mechanic, and even seemed to prove a narrative burden.

^THIS^ Minus Split Screen, which is not on PC.
On OG consoles it was the same aside from CE which was 2 players local or LAN but not online.
Yea, i just copied the MCC console details, too lazy to edit out the split screen
