left tooth missing in Home of the Forsaken

In Home of the Forsaken, I have found all of the fragments except the Left Tooth. The walkthrough says that it's on the table in the cave hideout you go to in The Last Stone. However, if you decline to side with anyone, you don't go to the cave.

I was able to find the cave afterwards anyway, but the fragment is not on the table. I know I did not get it, because the quest says I'm missing a piece, and all but this one are accounted for.

It seems like this is a bug, or perhaps a very annoying design choice. If anyone knows of a solution to this, please pass it on here.

Weird, it's not disabled by anything. It's always there.
Also it has the "havok dont settle" flag which doesn't make it interact with the physic system before touching it, so it can't be blown away by some explosion.

It's on a paper which lies on the table where the powder barell is.

Do you happen to use any mods?

I am not using any mods. In the cave there is a large table with a meal, a water bottle, and an alchemy setup. To its right there is an empty bench-like table, To its left is a fire container, and to the left of that there is another bench-like table with an inactive piece of paper on it. There is no tooth or anything else on this paper. I can find no other tables.
