As to when she's on the island, apparently she's bugged in regards to the dot not moving to show she's ecstatic. If you hugged her and gave her an apple you should have reached it. Apparently the box lighting up is what you need to look for, not the dot.
I know some people have gotten this achievement before this update. But I don't know if this update will bring any changes.
Hope this helps.
B) People have reported only getting this achievement successfully once they got Jackie happy enough, which implies that Jackie's mood is relevant (and suggests that Daria's and Beverley's are too).
Lily doesn't appear to have a mood, but the other three definitely do, and it appears to be relevant (though it's fairly easy to get most characters to ecstatic without noticing - particularly since the indicator dot doesn't provide clear feedback near the happy/ecstatic border).
