I finished the game today with the Lily storyline.
Spoilers following obviously.
In my opinion Stella is in a coma/in the process of dieing while the game plays.
All of the places of this Limbo are built after special events of her life, like the japanese looking region (she was living in Japan for a while) or the urban city region (when she moved to Vancouver [was it Vancouver?]).
The spirits she brings to the everdoor are also remnants of people that touched her in life. Friends, family members or patients from her work in the hospital.
They are there to soothe her and prepare her for her own death.
So it's her subconsciousness that's building the game that we play until she is finally able to die.
She gets glimpses of the real world when she interacts with the shrines and hears her sister talking to her and drinking Whisky.
And maybe that's also the reason why she looks so young, because in the inside she doesn't feel as old as she is (I can relate to this :D).
And for me Hades is the symbolization of Death that's waiting for her at the end. He is kind of beautiful and intimidating at the same time.
Just my 2 cents about this. ;)
I really liked the game.
