How do I get the different endings? Spoilers, obviously.

So, Just finished with the good ending for my first playthrought.
What are the requirements for the different endings? What can be done differently?

As far as I know there are 4 endings.

One ending is at the begining where you grab a gun in the sewers and shoot your dog like a heartless monster.

The last 3 are after you kill the mouth face thing.

Good end: Go left at the intersection to the more rundown section of the map and fight your way through to the end.

Bad end: Go right and abandon your dog

???? end: I'm still not sure how to get this one but I believe it involves giving the arm behind the locked door in the Good end route some sort of item. I'm not sure what to give it however.

Edit: So the 4th ending is the nightmare ending and you need to do 3 things

1: Take the doctors body from the lab and drop it in front of the lock picks

2: Kill the Siren (mouth face thing) and the thing that pops out. Leave the room and come back to pick up an item.

3: Give item to hand in the good ending path.

Huh, so I just missed the Nightmare ending because I somehow ran past the tree?
