Hellgates are for people who don't care about their sect alignment; hell rooms are for those who do.
A hell room is a room with horrendous feng shui - literally the worst you can make - that is also sufficiently hot or cold to quickly kill anyone who is in it for an extended period. Making rooms out of conflicting materials, with conflicting feng shui items and many doors, is one way to accomplish this. Hellgates are a simple extension on this idea that puts a sect gate in the room, so that any visitors to the sect (including the recruits you send to your sect from your city agencies) will automatically place themselves in the hellroom and die. People killed this way will gradually accrue evil rep (possibly from remaining unburied - not sure, haven't experimented with it), while hellrooms can also be used for rep-free anguish gems by simply dropping unconscious human attackers inside and letting them die there.
Hellrooms/hellgates also serve a secondary purpose that's specifically relevant to spectral refinement: They drastically increase your chances of success at refining items placed inside. For this reason, it's good to have a display stand or two that you drop items on, then remove and forbid so you can refine them at leisure.
Also if you don't mind "cheating", get mod that make refinement has 100% success rate. That'll reduce the anguish soul gem that you need to collect. Less people need to die.
With 100% success rate, you probably have enough soul gems by buying from other sects and putting dying attackers to die in bad feng shui room. Don't need to lure innocent mortals to die at your hell gate.
recently i mostly use the 100% cheat because dealing with hellgate grind got a bit boring.
