Where is Titanite?

I think I've checked all ores and junks...

I'm pretty sure you have to refine an ore to get Titanite.
If you mouse-over an ore, the tool tip will show what the ore refines to.

Titanite seems to come exclusively from refining Zai ore. At least I haven't seen it anywhere else.
