I prefer Extreme with no hud and x ray vision, but if its your first time i would suggest hard probably
I would suggest playing on Normal at first to get to understand the game and it's mechanics. Hard mode is better for a tense survival situations at first,. Not done Extreme yet not since the patch. So couldn't tell, though I bet it's beyond tough :p

Hard is a good level, if your play extreme turrets still work and will shoot you while hacking, but time is not stopped, so hack fast!

To make a real challenging game, after playing through a couple times do this.

Play on Extreme
Do not loot Energy cells from dead terminators, only Buy, Get from Crates or Make them.
Do not look more than 1 type of enemy weapon so no multiple Plasma Rifles
Do not upgrade Backpack to more than Level 2
Do not Make Medical Kits
Do not use StimSticks
Do not use any other weapons but the 2 plasma Rifles
Do not use Skill level in Stealth, Toughness, or Weapons
Only allow one of each type of Chip

I did the game with these settings and its much harder, you have use cunning by hacking turrets to let you do the dirty work and using Grenades.
Was wondering the same.

Many Youtube reviewers said it was easy, and to play on the hardest setting.

However I find it challenging on Hard, or the 3/4 difficulty setting. Have played 7.9 hours.

Tense though.
i play on extreme myself, that is the best experience, terminators terminate you when you dont have any cover and think youre Rambo and stand on the open field.

be tricky and use pipe bombs too

Hard. Otherwise the Terminator's aren't really that scary and the whole concept falls apart. I haven't tried Extreme yet, but Hard is a halfway decent challenge and def. not unfairly hard.
