How do I use the knife?
I've tried sneaking up on several T800s, successfully at that, but can't perform the kill with the knife.

Tried standing, crouching, moving in point blank range, pressing different buttons - nothing.
How do I perform the takedown?

Did you equip the knife? Standing behind a t-800 with the knife in hand, you should be prompted for a RT kill. It's not like your pipe to smash boards or how you throw grenades. You need it in one of your four weapon slots.

yep, equipped the knife, no prompt. maybe the angle was wrong, can't tell

Aim at the head, the prompt should appear when your close enough, you need to be holding the knife, not just have it in your slots.

Warning, will not work on T-600s. Or T-47s. I tried both. Works on all T-800 models though.

Just discovered the weapon with the "new" Annihilation Line DLC. Haven't yet had the chance to try it.

when you get close behind a terminator you should get an interact prompt. For a keyboard and mouse I think the stock control is E. Just push that. Personally, I don't use them at all. Only one time to get the achievement.

Usually, when you are in a combat situation, stealth really isn't an option. It only works well when you come upon one that is not in view of other terminators, and he hasn't detected your presence.

I mean, if you REALLY want to get rid of a terminator and your options are knife or shootout, then on higher diffs you definitely choose the knife. Unless you have a hacked turret or something nearby, anyways...

Edit: Also, yeah, the knife interact prompt (left click on K&M) completely disappears when the Terminator knows you're there. So they ONLY work when you're undiscovered - don't know if there's a lore reason for that, but it definitely sounds plausible.

Oh, how I WISH there was a patch that merged that DLC with the vanilla campaign. Weapons and T-600s and all.

From the animation, it appears that Rivers shoves the knife into a specific access point on the front of the T-800's neck. Perhaps that access point can only be reached by approaching the T-800 from behind.

Yes, I'm aware of that. It seems the knife is electrified, so Rivers inserts it into the terminator to fry its electronics from within. What I was referring to is the knife being unusable when the Terminator "spots" you, which could either be a gameplay consideration OR some kind of built-in safety where once it enters "combat mode" it isolates its internals to better protect itself or something.....

It COULD be that. The lore states that the T-800 and its variants are supposed to be more resistant to electricity and EMP attacks than the other Terminator models, as that's the primary infantry model that Skynet uses. Perhaps that part only activates when a T-800 enters combat.
