The base game comes with a set 3D theme for each category of songs except the 3 DLC categories ("House", "Universe" and "Japanese Cyber"). When you play a song in a DLC theme category without owning that theme, it will use a theme from one of the base game categories instead.
You can also select from multiple 2D themes to use, regardless of song category. The base game comes with, I think, 4 different 2D themes to choose from. But if you prefer 3D themes, the only way to have access to multiple ones (and the choice to use the same 3D theme for every song category) is to purchase a theme DLC.
Are you still with me? I hope I didn't make that too confusing.
MUSYNX has 4Key charts and 6Key charts and on the higher difficulty songs you will at times have to press down 4 keys and 6 keys respectively, at the same time.

I recommend you give the base game a try first, before thinking about buying Forever Friends or any of the theme DLCs. The base game is really affordable and comes with over 50 songs with 4 charts each (4Key Easy, 4Key Hard, 6Key Easy and 6Key Hard) - for a total of more than 200 charts to play - and fully functional leaderboards.
