You are missing the replayability aspect of gaming that some people enjoy... Lots of us enjoy having a persistent world that we can roleplay in that is on our own computer and not tied to a server that someone else controls and can end at any time...
For example, I had a ton of killer mods installed on Skyrim just so I could have an army of hot followers if I so chose... Now I didn't do that when I first played Skyrim... But once I had played several quest chains through to the end, I installed everything I could that wouldn't crash it...
The reason I did so is that I wanted to have a game no one could take away from me where I could do whatever I wanted, whenever I wanted, with whatever NPC's I wanted to... I could add locations and live the life of a Mercenary/Sheriff/Pirate/Bandit/WealthyLandowner/DirtPoor farmer/slavetrader/ and the list goes on and on and on and on...
I had a vast variety of role playing starts that I could jump to on any occasion that suited me... If I had a day or week where I was free, I could jump in game and play as anything I wanted to... Marry whoever I wanted to... Kill whoever I wanted to... Etc etc etc...
Without all of those killer mods I would have been stuck with the base game that I would have played through once or twice, maybe 4 times at most, and never touched Skyrim again... Instead I got years and years of playability out of a game that would have other wise been a waste of time and money...
check it out, it's super easy to install, all you need to change is the keybind in the mod menu.
While I agree with mods extending the life of the game is a good thing, nothing in that list I made adds to replayability.
I, too, enjoyed a gaggle of hot followers in Morrowind... when I was 13. These days I mod for content, not vanity... or fap material.
I sure didn't need to know what you did while gazing at toons as a 13 year old...
Or that you still to this day consider hot followers "fap material"...
I'm going to need bleach to clean my eyes now...
Please and by all means Feel Free to refrain from speaking about what your naughty self did while gazing at your toons in any further communications with me...

Good looking NPC's and Killer looking Weapon and Armor mods, along with mechanic changing mods in an open world game has zero to do with "fap material" or "vanity" for this part time gamer...
I cant speak for the rest of the world though, as you have so clearly and unfortunately illustrated... For me, it is content...
No, it's absolutely vanity still. It doesn't enhance the game in any meaningful way.
Not saying don't do it. Just say it like it is.
To me the idea that a grown man would spend extra time making people in a video game prettier is odd to me. It's truly not far from playing dress up in a nerdy way. But then again, I guess even weebs have to get older.
For you maybe... Not for everyone...
A lot of these mods you obviously consider vain add tons of replayability for me... For my playstyle, It adds to the replayabilty aspect of various games to have lots of options to choose from in future playthroughs...
That includes those that you consider vain... To me, it's just different looking NPC's than the base game has, along with more options to interact with the NPC's than the base game has...
I get it though... For you, a lot of these mods are not an option that you find enjoyable...
Just realize that for others it is what makes it enjoyable...
