The only thing I can think of is difficulty. I play that save on easy, perhaps they survive more then.
So what I try to do is keep them fed at 100%, wait for the note telling me to do something else, sleep to bring up wakeful percentage, wake for feed and note telling me to do something else, short sleep, wake to feed and wait for note, rinse and repeat until the pup recovers or dies. If I must hunt, I try to do it right after the note telling me to do something else.
However, it is a common misconception that keeping the pup full (100% or something like that) helps them recover from sickness. Hunger and sickness can both kill a pup, and if both affect the pup they will die faster, but hunger does not affect the pup's chances of recovering.
Just wanted to mention because I know some people think keeping the sick pup at 100% hunger helps them recover, when in fact the only way hunger affects sickness is if they start starving or become too sick to eat.
