The game lags when I launch it, when I aim with the right button it stops lagging. What do I do?

I'm OP's friend and we tried the following:
- lowering all graphics settings (including disabling motion blur)
- switching resolution to a lower one
- switching from fullscreen to borderless
- switching to DX11
- closing Nvidia Geforce
- updating graphics drivers
- reinstalling the game
- restarting the computer

He also has well over 100GB free on his local disk where the game is installed. Other games work fine,
In Back 4 Blood fps drops are constant when he moves his camera, both in the hub and during the mission, making the game completely unplayable.
The most curios thing is that there are no fps drops if he is aiming with RMB, as can be seen in the video.

Any other ideas about what we could try or check?

Why don't you write down the computer's specifications, maybe it would make things easier. There are two sounds in the video, something is connected or disconnected from the computer. Does he hear these sounds all the time?

even people that don't own the game

If NOT, are there any reasons why you don't have write permissions, other security software, etc.

Got an avarage rig, playing on ultra with 4k resolution and in Fort Hope I get like 275 fps....
Game is miles away from being demanding.

Reality:
a bunch of lvl 0 profile gamers can't play for whatever reason.
Meanwhile: 11k players ingame.

Alt accounts can have their profile public. But who buys a game for their alt? I’m calling occham’s razor on this one.
