Which I did. Must I beat the whole of Chapter 1 in one run, starting from the very first level of the first chapter?
I'm playing online.
Did you play with friends or quick match with randoms? The reason I ask is that if you missed a stage then you will not progress.
Quick experiment - go to campaign - start run, select difficulty and select act 1 - below that you will see select starting point - click on that and you will see what checkpoints you have unlocked - if all are unlocked until end of act 1 then your game is bugged.
Another possibility - if you did tutorial and then continued with the story the tutorial part does not count as part of campaign (several people have reported this) - you have to do the section that is included in the tutorial in the actual campaign.

Thank you, the starting point menu tells me exactly what I need. For some reason it didn't register that I've finished the last 2 chapters (Abandoned and The Sound of Thunder), but I just started on Abandoned, cleared both levels and unlocked the next act.
