More than half the posts here are about the controls. Mainly that there is a delay in swinging your weapon. If you tap the button, instead of holding, it should still swing the weapon. If you jump you should be able to attack as well and when you duck, you should simply be able to press down and then the attack button, not a whole other button dedicated to crouching and using your weapon.
