I figured out how to get the secret ending.

I'm not exactly sure if you need to do both, but in my playthrough I got every file and did the 4 pyramid puzzles in the otherworld. I think you have to get every file though because of how this ending triggers. After the final boss fight and the cutscenes instead of running towards the right when Paul says lets go, run back up the stairs towards the arena where you fought the boss. The Master's body has the second to last file on him and once you pick it up the 5th red portal appears. Go in it and you'll be able to do the 5th pyramid and unlock the magic book which is the very last file for the achievement.

As far as the secret ending itself goes, I highly HIGHLY recommend you experience it for yourself first because I thought it was absolutely amazing. But if you don't care, I'll just summarize what happens. After the last pyramid is finished, you go through another blue portal and a cutscene plays showing Alicia appearing in what looks like a jungle with Regina from Dino Crisis's outfit on and sure enough she runs into a trex. This also unlocks the Sci-fi Commando costume which is basically just Regina's outfit.

I confirm that all the files are needed, since I seem to have missed something, since after picking up second to last file, the portal did not appear.

Collecting all documents is not necessary for getting this secret ending. I didn't unlock the "collect all documents" achievement but still got this ending.

Just as what this post tells, I picked up the file on the corpse of master and an portal showed up automatically. I tried it for three times. Collecting all documents is definitely not the trigger condition.

Yep. In addition to collecting the rest of the files and decrypting them on this playthrough I also unlocked every room in the mansion, got the 100,000 dollar key, and got every weapon so something else could possibly be the main trigger.

Yeah I've done several full playthroughs at this point on different difficulties, camera modes, and levels of completion and having seen it. I haven't done a 'kill everything' playthrough yet though and I don't think I have every single note on one playthrough either. I haven't gotten the key yet either.

Also, there's still one ending missing. There are 5 epilogue files but the only endings I've seen mentioned on this forum are: "Very Bad", "Neutral", "Good", and "Secret".

I tried again, using that strange device to decode master's diary is a must. No using, no extra portal.

Speaking of portals... in the spirit world, on the way to the 4th pyramid, there is another unlocked portal that leads to a locked portal. I splashed it with blue water, crossed it and... there was nothing there. just 2 creepy crawlings and a small area, very similar to where the "pieces" puzzle (the one you have to solve during the quest for the very first pyramid ) was , but without the lever or the altar for the puzzle.

I thought it was irrelevant so I reloaded the save and completed the game and only got the "good" and "bad" endings.

Maybe it wasn't irrelevant...?

Alicia can gather several pages which compose the document called "Unknown Source Diary". These pages have strange unreadable symbols on them and are scattered on Fog Island.

In the cathedral on Skull Island, there's a strange device in one room which needs an old key to unlock. You can get this old key after killing a zombie in another room (should be a dormitory because it contains many beds). Investigate this strange device for five times and the unreadable document will be decoded into "Master's Diary".
This is NOT the same doc that you find on the corpse of master after the final battle, but it's required for making the final portal appear.

The biggest clue I could find to what that portal is used for is the Kid's Drawing file you find in the room that also has the dog's (Yume, the dog thats in the room you open with the 100,000 dollar key) collar in it. I think the fountain in the drawing is the one right in front of the Green Emerald door but still not sure how to decipher it.

I got a secret ending. I received a translation of an unreadable text through the recorder. Then I after picking up second to last file from body of Master and teleport appeared. True, I was also given an achievement that I have collected all documents. I will share my suspicion with you:
1. Collect all pages with unreadable text.
2. Clear all yellow portals and restore all pyramids in the game.
3. In the master's castle, translate unreadable text through a recorder.
4. Picking up second to last file from body of Master.
5. After the appearance of the portal, go into it and restore the last fifth pyramid.
6. Read the book and enter the newly opened portal.
Congratulations, you've got your secret ending.
P/S. My guesses have not been confirmed yet.
