I just re-started and spawned in a different location each time, so not sure what your issue is.
The only spawn i do in this game is when i start clean new game. Then is just save and reload.
If you die and choose to revive you will lose some items from you inventory and durability from the ones you still have and you will be respawned at the starting shelter if you don't claim a cabin/ white house as you base, or build your own base. If you die and choose to revive go back as fast as possible to the place you died and you will be able to recover some of the items you lost. Is never recomended to revive if you died, just save often and reload the game.
I never died in 81 days but i reloaded the game a bunch of times because i got stuck in stuff, bandits destroyed parts of my car while passing by their base or my car flipped or got stuck in ditches or trees. Saving often and reloading when you need to is a great strat.
