There are two events in the apartment area that trigger something truly evil (STE). The first is the event with your neighbour 000, if you select the last option 'w-what is that photo' you've triggered STE. The second is the mail event, where there's a random chance that the letter you open triggers STE.

Once you've triggered STE, several events around the game will be replaced with STE variants. From memory there's the pale woman painting in the apartments, as well as the box in train from downtown. These events add 1 point to the STE counter, with STE appearing once you reach a counter of 4. There's probably a few more events that I don't know about, but I do know that with a high enough STE counter, resting causes STE to appear and raise your counter.

Like I said above, once you reach STE 4 she appears. She's a random encounter like any other enemy. Just wonder about several locations and she'll eventually jump out. Presenting you with a very unique fight.

You're better off. She's a run killer if you're not ready for her. Like Dr Chocolates said, the only guaranteed way to "trigger" her attention is the neighbour 000 event and select "w-what's that picture? . .". Rolling a natural 1 on the luck roll in the letter "Mail Day" event seems to also trigger this. Note once you get her attention resting at home becomes very unsafe. I got 2 counters from resting alone (she shows up in your computer screen and the message "Something evil knows where you live" pops up). Once the counter reaches 4, you have 1 chance to recover and ready yourself. The next investigation will always be her encounter.

Her fight gives no experience to my recollection, and I burnt somewhere around 10+ STA and 5+ REA on her. You can't fight her; you can only harm yourself in hopes of appeasing her while she is trying to kill you. After doing enough damage to yourself, an option will pop up to end the fight. Once you "defeat" her this way, you get an achievement and the counter resets to 0.

Edit: Actually it turns out you can fight and kill her with enough spells like Ithotu Flame since they get around the combat actions limitation her fight presents. You get 60xp for killing her, but the counter does not reset, so the next investigation will be her again until you appease her by self harming. This may get patched later. I think that was patched so that you can't fight her again.

She's a nice piece of set dressing that does a good job of coming across as sinister, but she doesn't really do anything unless you go out of your way to provoke her, and there is very little impetus to do that other than to see her events.

When you do end up getting your counter high enough, if you can spare the doom then you can rest as many times as you want. Every time you rest your STE counter goes up one, but you'll never actually encounter her in any of the locations like your home or the monument, so can rest up entirely safely.

When you do encounter her, suffer for her and cry for her 4 times between the two actions and she'll mellow out and let you get away, resetting the counter to 0, which in as far as I can tell doesn't increase unless you visit Junji again and actively look for her, or get shockingly unlucky with the mail.

Moral of the story: Junji Itou is kind of a ♥♥♥♥ for keeping that picture around.

Going past 4 counters delayed her appearance for me. I don't understand the logic behind her appearing in counter 5; didn't see what happened past 5. I got 5 counters on endless and continued normally. I leveled up and was able to investigate normally beforehand. She appeared out of nowhere 5 minutes later in a random investigation.

Also seems like you can purposefully get her attention and eventually fight her again like Detahramet said by neighbour 000 event and saying, "w-what's that picture? . ."

I think that was patched so that you can't fight her again.

I started a run last night where this was not the case. I killed STE and had to fight it again a few turns later.

I think she only appears on the next combat encounter, not just any encounter. She seems to only deal a measly -1ALL while being eldritch, so Abolish works wonders. Too bad you can't throw stuff at her.
