Missing Codex Entries

I've basically finished all the quests (except getting the outpost fully functional), there's only one that's bugging me. Recording knowledge on memory crystals. I have 211/221 codex entries and, as far as I'm aware, have talked to everyone on the map and explored all dialogue options. I've also bought all the books (the normal and the rare ones) that I could in Larnak. The only hint I've seen of something else I've not completed is where the screaming wind comes from, although I've had so many dreams and hints it's all pretty obvious. Oh, and I've been at -1000 with the abolitionists all game so if they any quest lines, I haven't seen them. Other than that I've done all the companion quests, all the bounties, everything else that has ever come up in my log.

The missing entries are:
Smolderbone Flats
Deven
Tectum
The Golden Age
Iyawabi
Constrictor

On top of that, the trade comparison shows two locations that I've never seen: Onyx Outpost and Stygios Lumber Camp. Do they even exist? If so where are they? Ordering things by distance always places them furthest away from everywhere.

Any hints on how I could fill in the blanks would be much appreciated. I find Deven particularly odd as I must have been there hundreds of time.

Iyawabi - is when you talk about sth with with Dhuzani from Last oasis, but protagonist need to have enough points in herbalism

I'm most confused about Onyx and Stygios tbh. I've literally walked around the entire border of the map, and no sign of either of them...

After Larnak my codex entries are 214/221. But i.m only in 222 days of voyage.

Onyx and Stygios must be some kind linked with one of 3 trading houses. Just a thought, may be lvl 5 of faction standing trigger new entries.

As far as I'm aware neither Onyx Outpost nor Stygios Lumber Camp are visitable. Might be able to find them on the prologue map since that has most points of interest filled in, even beyond the area playable in the open world game. [edit: Nope, only major landmarks.] The market simulation in game models towns beyond the boundaries of that area as part of setting prices based on supply and demand, maybe these two aren't properly marked to be hidden.

Stygios Lumber Camp and Onyx Outpost are 2 towns in the Sunfire and Moonshadow expansion. I hadn't seen anything regarding those until I got the expansion and travelled to Xeryn, the new "continent" north of Avernum
