You have to buy the base game.
Any DLCs you bougth in 4th wil be yours for free, when they are released. The first one will be the Reaper somtimes in the near future.
The 5th is reworked for shorter games and has some/a lot rulechanges, cardchanges and fieldchanges (and acomplete rework of artwork)
For Bugfixes, this is an complete new engine and has bugs. Old and new. And less bugs (old). But it is in constant development and the devs listens to us.
And, btw, the RNG is no bug. It works fine here and in the 4th.

The AI is a bit wonky for now. Some decisions of it are rigth out stupid (killing self, alchemisting magic items if on the field, where they do not work, getting rid of the poltergeist, spending Fate to get Fate etc etc)
