That's because it's a bug. The game by default can't see proper VRAM capacities on current GPUs and even if you try setting the game to Ultra, it defaults to Low textures. You need to have the proper .ini file for the visuals to look as they should.

If you want Steam overlay and playtime tracking, just unpack dhewm3 into your Doom 3 folder, delete doom3.exe, and then rename dhewm3.exe to doom3.exe.
I'll be honest, I tried Dhewm3. There was zero difference to regular Doom 3 for me.
Dhewm is just as moronic as Doom 3 at default settings and both need .ini tweaking to work as expected.
I didn't have any problems with it.
