I just wanted to share it here, because it was actually quite tricky to find and I haven't seen the answer here.

The fourth part of the treasure is located at the lake at the hospital.
The suitcase is hidden between two big rocks on the side of the lake, near to the fence.
You don't have to look in the water or dig something up.
