If it doesn't work try disabling the steam input API from the game properties in Steam.
Press Start in windows. Type in 'set up USB Game Controller'
Goto the properties for the controller, and see if the axis and buttons are working. Sometimes the controller is detected but not functioning.
If it is. Hit advanced, and make sure preferred device is set properly.
If you are still having problems. Open up your device manager. View > Show Hidden Devices.
And clean out your USB devices. Uninstall all the grey devices in the list. If you see one with a device name that looks like your controller, unplug your controller, (It should turn grey in the list) uninstall that device. Restart your computer. Then plug your controller back in.
If you're connected with blutooth, same deal, delete and restart the pairing.
I usually have to go through this process due to incorporating UCR + vJoy for games like AC7. It completely trashes windows controller support once you try to remove it and leaves you with a non-working but detected controller.
Just noticed the DualShock4 comment. Maybe look into DS4Windows, and ignore all that steam input crap.
You could also try UCR + vJoy, just .... heads up.
