reqs iron and thunderstone, so after you enter swamp/crypts and find merchant in forest
Honestly I did this myself a few times before I started to pay attention closely to what I was doing. The button even says craft for crafting and upgrade for upgrade with text above the button to say what it is upgrading to..
The problem comes in, I think, due to the colors being all the same so it is easy to not pay attention to the menus.
The best solution would be to insert a message asking... "are you sure you want to consume x and y items to craft z" that you have to click an additional OK to for crafting.
That may seem awkward adding in a confirmation each time though.... Another more elegant solution might be to have the words "craft" or "upgrade" flash another color when you mouse over the button to pull the eyes toward the identification.
Adding these would result in the portal's restrictions being bypassed by simply crafting the most metal efficient equipments to bring through it, and resmelting it on the other side.
I forgot about that, you're absolutely right. However, if the materials you get back would be equal to either half of the material cost, or the cost of upgrading the item at its current level (which I assume is half either way) then you could technically bypass the restriction, but you'd only get half the materials, which would, at least to me, be a hard pass.
