First I will throw some guesses to help. Are you sure the names a correct, upper case, lower case, spaces all the same?
Are you sure the portal on the mountain was not destroyed? If an enemy spawned near and you didn't take care of it it may have destroyed your portal. Are you using the correct portal at your base?
Secondly let's get this story of yours straight, when you approached the portal at your base your saying it didn't create a wormhole? Have you returned on foot to the portal at the mountain and found it also not creating a wormhole? And you have confirmed they are both still intact and have the same name?
Otherwise I would not be able to transport myself to the mountains before I died.
Portal in the mountains are not destroyed as there are no creatures on the top most area, before I went down.
After I died, the portal in my home is just suddenly unconnected and no wormhole effect whatsoever. So the pain of sailing back to the mountains far away is the hard part due to this bug or glitch.
Anyone have a solution to this as it can happen again. Hopefully not.
If you think it's a bug, enable devcommands, debugmode, god mode and go fly there to confirm everything is as it's suppose to be.
Prove it's not destroyed and confirm tags are the same.
