Howling Caverns Boss Missing?

I'm playing on a friend's server. We entered the Howling Caverns and died against the boss. We went back in a few times and failed to kill him. Then we re-entered, and he was dead, leaving behind the clothes chest. So... natural causes?

Disappointed at missing the fight, we then went to another Howling Caverns instance. The boss didn't show up at all! Is there supposed to only ever be one of him despite his ability to show up as a raid event?

There is a bug on current version, which is responsible for only one miniboss from each type spawning. Once it spawn on one location, it won't on another. Will be fixed on next patch.

I couldnt find mini boss either. There is no secret breakable doors left in this "?" mark.

I've also had an issue where the miniboss in Howling Caverns spawns in one of the holes in the wall, so you cannot access the miniboss at all but you can barely see him there.
