Constantly attacking.
After login my character starts punching the air and will not stop. Cannot play the game because you cannot do anything except punch nothing.

No stuck buttons I checked for that, not sure what you mean by verified files?

also reboot, log on log off, change map. No idea at this point

let me make one minor correction, I can open the inventory and work in the inventory, this pauses the punching. If I come to a stone with inscription while punching it I can read it but it continues to punch while reading.

For me when my char does this I hit the attack key myself, and it does that hit and stops. I have this problem occasionally.

yes I have tried that, as the attack is my LMB I even disconnected my mouse and reconnected thinking it may be something with my mouse.

Sometimes, I have this problem when my Xbox controller is plugged in. (Even if I do not touch it).
Everything is back to normal if I unplug it.

I had the same problem. I had my keyboard and mouse, controller and steering wheel setup plugged into my computer. After unplugging my steering wheel the problem stopped and hasn't returned.
Do you have other peripherals in your usb slots? try changing your mouse to a different Usb slot it that is possible. Not sure if it will work but it worked for me

Tevirik, you are a genius! I did not realize I had my Saitek x52 plugged in as well. As soon as i unplugged that controller (even tho I was not using it) the issue went away. Thanks for the response!
