Mistlands on an old map
I started Valheim approximately 1.5 years ago and have discovered about 15% of the globe face when looking at the world map. I have a couple dozen portals and have found bonemass etc.

With the Mistlands update can I expect to find MIstlands somewhere on my existing map or will I need to start over?

If you go to unexplored region of mistlands, it should be okay. It may be slightly different from the intended experience, but on my own part I can say that even older world seeds should have approximately same amount of features as new worlds.

It depends upon how close you explored to the Mistlands when you last logged in. I'm about where you were, perhaps 20% and only in a single exploration path, as my Boss stones were somewhat aligned....and I've found plenty of Mistland content, not on the areas I exposed earlier, only get the mobs there, not structures, mines, POIs[AND NO MIST!!]...it wasn't that much, only a few small biomes and a good sized one on an island. It really depends how much fog uncovered in a Mistland biome area.

I have he same thing as you OP. Still using the map I set up at my start a month or so after game release... still not explored it all (OK I may be slow but why rush?) You'll find some have no doubt!

The "Upgrade World" mode has been my go to after every release of content and it does it exactly what it says on the tin. In fact considering using it to reset & refresh my world (bar the buildings) and inviting friends over ;)

Did not know that previously explored Mist Lands would be absent of mist... that is interesting.

I've now done three play-throughs of the Mistlands zones, with two being "old" maps and one a full run starting after Mistlands came out. Based on what I have experienced, they are largely the same. The only differences seems to be that areas in the "old" maps that I had discovered as Mistlands previously are now sort of semi-Mistlands. They have some mist, but nothing like the dense mist of modern Mistlands areas. And they have hares and sometimes the other creatures, but no other resources. No bones, skulls, Infested Mines, Dvergr outposts, etc. The landscape in the in the "semi-Mistlands" areas also seems far less rugged and vertical than the modern mistlands.
Just to be clear, on the "old" maps, when I travelled into undiscovered Mistlands areas, they look the same as in the ones I found when doing a full play-through after the release.
So if you want the modern experience while using an older map, go find Mistlands areas that you had not previously discovered.
