He will show up when you are in rendering range, which is a lot further than sight range, and beyond what the map will show as explored. The bag icon will be added to your map, likely still in the fog of war area.
Once he shows up in a given world, his location is fixed in that world. The other 9 spots are disabled.
He spawns in a circle around your Forsaken altar. He may be close, he may be far. But he is always within a certain radius of the center and always Black Forest. I notice he usually spawns in BF zones near swamps. So if you are seeing plains, you went to far out and need to circle around another way from the center.

The best strategy it to sail around and hug the coastlines of any black forest biome you see. If you're lucky the bag icon will pop up on your map. Sometimes he's too far inland for the bag to appear, but sailing usually works and prevents you from running around as much.
Otherwise every thing could go by this rule.

I always force it close by using a circle around the center
