yea my first play-through i had it outside and i decided to try it indoor.
I find constructing in the game a bit strange.
anyway a 3 wall high wooden wall/tunnel for the smoke to go up and a slanted roof so it doesnt go out in rain should help get the smoke out and the heat in :P i think if the wall is only 2 or 1 high the smoke will choke the fire
Here is my ventilation. Handles a hanging brazier and a hearth with exactly zero issues.
does pillars work when you place them on top and place them tell they hit ground? or does the pillars have to start on the ground?
random sidenote, the longer smoke stays trapped the worse my performance becomes, its my biggest fps killer/overheater

What you want to do is build a regular house of your design and then add in a dedicated fire place with a chimney.
