Achievement read all files

Does anyone have a problem trying to get read all files achievement ?
I already took all of them but is still missing one. 57/58

The last file i found on my run was kind of hidden in plain sight. It was on a monitor in the room before the greenhouse and it required the senior staff chip in the security bracelet to access.

I was in the same situation. The document I was missing was the letter the director in the upstairs bathroom of the orphanage. It's pretty out of the way and def one easy to miss.

I found, was the sewer 1 inspection playing with Ada. I have taken this alread, maybe didn't save properly before.

You only need to check the document once. The game remember you opened it anyway. You can check your status in the menus.
Anyways, it seems you've managed to achieve that success so don't bother....

I was missing two documents as well and I went on a website that listed all the documents and their location. Fortunately I had a save for each variation which helped me grabbing in no time the missing files.
