There's a bug in the game where if you change the 3D Audio setting, it will break the audio. You need to relaunch the game if you change that setting. Bug is also present in RE3.
Enabling the 3D Audio option on its own will not do anything. For it to have any effect, you need to right-click the Sound tray icon in Windows and set Spatial Sound to "Windows Sonic for Headphones". If 3D audio is off, then the atmos isn't even activated. You're playing in normal stereo.

Yes, but like I mentioned before, that is not enough. For Atmos to work, you need two things:
1. Atmos activated in Windows sound settings.
2. 3D Audio turned ON in the game's settings.
Ah, see that's why I was confused. Every other game doesn't need that extra step.
RE4 Remake is actually like that. There's a 3D audio option but you cannot change it. The game tells you that if you want to turn it on, you need to activate Spatial Sound in Windows.
I just finished a long round of testing again and I can confirm that Dolby does NOTHING if 3D audio isn't ON. Here are my findings:
Spatial sound OFF + 3D audio OFF = Normal Stereo
Spatial sound ON + 3D audio OFF = Normal Stereo
Spatial sound OFF + 3D audio ON = Virtual surround
Spatial sound ON + 3D audio ON = Virtual surround that sounds different.
I was surprised to see that having 3D audio ON without Dolby or Windows Sonic gives you virtual surround. It seems the game has its own virtual surround implementation.
