Its not definite. I think it only occurs if you are being actively chased or if you make noise in that area

Hall is not safe room. If you have original soundtrack you will no which rooms are safe rooms.

A friend was watching me play for the first time when I encountered him near the chopper. Ran into the main hall in panic. The friend asked if he could follow, I said 'he shouldn't as this is a safe room', and at that exact moment the bastard blasted the door open)

He cant follow you into the tunnel to the basement, which, I suppose counts as a safe room. And to that extent, I assume he also cant follow you down the stairs to the basement either

You most likely didn't collect the herb next to him, otherwise he gets up if you do.
