Either picking up or using ink ribbons, med sprays and special weapons and also time should be factors.

0 saves = 100 points
1 save = 90 points
etc.

Unless you go for s rank, you should be fine though. Haven't found any official numbers for the remake yet.

Untrue. Beat the game under 3 hours and 30 mins to get an S ranking. You can save as many times as you want.

That was the old system. As far as I know the only thing that affects your rank is time.
