The early Tr 5 is due to some major changes in the core of the sim, affecting some missions. This will be corrected but I will need to complete the core features of Apollo first.
Keep in mind that the IVI's need to be corrected at Ap or Pe, relative to what you wish to do. But in this mission, it should be OK once you hit orbit. However, as the sim is different each time you play due to the physics involved, the achieved orbit is the prime factor of your burn.
In many cases, you can circularize directly, but it's hard to get it 100% (and is not needed). Try to do this at Ap or Pe, and it will be much easier.
Additionally, when it comes to reentry, it appears a very important part of the checklists are lacking. Specifically, there's no prompt to start the event timer, which is not a huge deal, but larger if you utilize the checklist "run" function rather than simply reading it, you miss the indication that after retro you should run the BEF checklist, which leaves the craft at a bad angle into reentry.
What situation is this in?
