What are all the requirements for it? I know each faction has to be done in different order across at least 3 playthroughs, but does stuff like which Dark Event you choose also effect it? Any others?

Each faction simply needs to be picked as the 1st, 2nd, and 3rd choice through each of your campaigns.

The Progeny -> Gray Phoenix -> Sacred Coil
Gray Phoenix -> Sacred Coil -> The Progeny
Sacred Coil -> The Progeny -> Gray Phoenix

Even after doing the prerequisites, I didn't get the achievement. The order in each campaign I took was:

Phoenix/Coil/Progeny
Progeny/Coil/Phoenix
Progeny/Phoenix/Coil
Coil/Progeny

the dark event you choose does have an effect as well though, if you want to see all the story dialogue e.g. Jane Kelly's talks.

I played
normal difficulty:
Sacred Coil -> Gray Phoenix -> The Progeny

impossible difficulty:
The Progeny -> Gray Phoenix -> Sacred Coil

Progeny = 75%
Gray Phoenix = 50%
Sacred Coil = 75%

So what I think is you need to do is play the game 6 times.
The Progeny -> Gray Phoenix -> Sacred Coil
The Progeny -> Sacred Coil -> Gray Phoenix

Gray Phoenix -> The Progeny -> Sacred Coil
Gray Phoenix -> Sacred Coil -> The Progeny

Sacred Coil -> Gray Phoenix -> The Progeny
Sacred Coil -> The Progeny -> Gray Phoenix

You get 25% from completing it at all three points in the timeline, and then the last 25% comes from the dark events, as the one you pick determines what missions you do.

I just finished a playthrough with Progeny/Coil/Phoenix. I forget which exactly were the Dark Events I choose but right now they are all sitting at 41% complete.

Are there only the same 2 Dark Events to choose from regardless of order done?

No, the Dark Events are random.

AFAIK each faction has 5 dark events. Each time you complete an investigation, other two factions complete one dark events (so third one will have two dark events running). You start investigation by countering one dark event(randomly chosen), then choosing one more to counter and one to let happen.

The order is defiintely not sufficient, I see this lie spread out a lot. I did 1 2 3, 2 3 1, 3 21 which should cover all the bases but still didn't have the achievement after that.

Dark event maybe, but you just have to keep getting to the final fight for each faction and get lucky with the variation I think. I have the achievement by the way, took about 5 or 6 playthroughs.

You didn't do 1 second or 2 third in that setup, so no you didn't cover all the bases.

At least that is my theory/understanding of how it works.
I'm still not sure it is correct though as people keep rambling on about this dark event nonsense.

I think it has to do either with dark events or the ^^stroy quest reveal missions^^

Except for the dark events
i know i only did the ^^side missions^^ for Progeny
one on my first playthrough
and one on my last cause i was out of stuff to train or assemble so i wanted to speed things up.
I countered the codex dark event and recieved/ stole some archon launch codes as a side missions. The Progeny takedown (as 3rd)went without codexes but archons instead.

Is there a way to see how much % you have without going to a new play for the recruit phase?

It seems nobody knows, because the global achievement stat for this one is still 0% while every other one has been unlocked.

Dark events definitely seems related. I just got the achievement and made a point of picking the various dark events. There is an extra one if the faction is completed 3rd in the order
